Exactly what the label says! Be it weather you are 3mg 6mg or 12mg. Add the numbers and they all come to 250ml of juice, depending on the Nic strength you want.

iirc 50ml is the amount of concentrate to make up 250ml of liquid adding nic pg & vg.
