For me the POTV calculator is ideal. They've thought of everything, by which I mean you can specify every conceivable variable, including the VG/PG ratio of your nic. And you can permanently save your recipes on the site.
The only way to know how much flavouring to use is to experiment, partly because taste is subjective and partly because there are many suppliers of flavourings and they're all different in terms of how concentrated the flavour is. If I'm using one I haven't used before, I usually make a small sample using the manufacturer's recommendation, but I've been at it a while now and I'm getting quite good at determining how strong a concentrate is by smell.
