The POTV calculator is a good one. It also allows you to set your nic ratio to 50/50 and is quite straight forward to understand. Your nic strength should go in as 18mg and you want to mix to total 30mlas that's what bottles you're getting in your kit.

As others have asked it would be good to know what make the concentrates you are getting are. Different manufacturers make them at different strengths so you might over or under flavour your mix depending on the make.
I see you're ordering from Vapable so I assume that you're using their own concentrates. They're usually ok at about 15% but you've got menthol which is usually ok at 2 or 3% in a mix so I'd suggest for your first 30mls (as you're getting 30ml bottles) that you try the blueberry at about 13% and the menthol at 2% of the mix and you can up the amount or decrease as you like to get what you prefer on the other 3 bottles.
I hope this makes a bit of sense to you. It can be a bit daunting at first but once you get it you'll be banging out bottles of lovely stuff.
